We Provide Quality Home Health Care Services

Employment services
Help individuals with disabilities find and keep meaningful, community-based jobs. Support may include job exploration, skill building, job placement, on-the-job coaching, and help maintaining employment. Services are tailored to each person’s goals and provided in line with their Coordinated Service and Support Plan (CSSP).

24-hour emergency assistance
24-Hour Emergency Assistance provides on-call support for individuals with disabilities who may need help during a crisis or unexpected situation. Staff are available at all times to respond by phone or in person to ensure the individual’s health and safety.

Night Supervision
Provide overnight care and monitoring to ensure the safety and well-being of individuals while they are sleeping. Staff remain awake and on-site to respond to the individual’s needs, which may include assistance with mobility, toileting, behavioral support, or emergency situations.

Homemaker services
Homemaking/cleaning services include light indoor housekeeping tasks for the specified client, not the entire household.

Respite care services
A person is eligible to receive respite if the primary caregiver is absent or needs relief from his/her caregiver duties. See the following sections for additional program-specific primary caregiver requirements.

Individualized Home Supports
Individualized Home Supports (IHS) provide direct support to individuals with disabilities to help them maintain or increase their independence while living in their own home or community. Services focus on skill-building, daily living support, and community integration based on the person’s unique needs and goals.
